MaxiCare is an AI native EMR platform built for operators of skilled nursing and assisted living facilities. The tagline says it plainly: the EMR that thinks. We are an early stage company building product alongside our first cohort of facility partners. The team is small and the work is real. What we build decides whether a nurse spends the shift charting or caring for residents, and whether an operator gets paid for care already delivered.

We're hiring a Full Stack Engineer who ships features all the way through: frontend, backend, database, and sometimes the infrastructure underneath. The platform is a HIPAA compliant, FHIR native EHR written in TypeScript end to end on AWS. Hono on Lambda, React 19 with Vite, Drizzle, and tRPC alongside REST and FHIR. Every choice in the stack was made to get the most out of AI code generation, and the team ships at a pace that only works when Claude or Cursor is part of how you think.

Compensation: Competitive base compensation calibrated to the experience you bring and the market in your jurisdiction, plus performance based annual bonus participation. Specifics are discussed during the interview process.

Remote: Fully remote within the United States, with a few hours of overlap with US Central time on most working days. You'll join the on call rotation, because the system runs in real facilities. Cameras stay on for every video call. Reports to the Lead Product Architect. Full time; employee or independent contractor status depends on your jurisdiction.

Responsibilities

Feature Delivery

• Take user stories from ready to shipped: backend handlers, database queries, React components, and the integration tests that prove it works
• Own the whole feature rather than one side of the API boundary
• Stay close enough to the deploy to know when something you shipped misbehaves

AI Driven Development

• Use Claude and Cursor every day, with prompts that produce code you actually trust
• Know when to keep what the model gives you and when to throw it out and write it yourself
• Share what works so the whole team gets faster

Code Quality and Review

• Learn the codebase patterns in your first two weeks and write code that fits them
• Flag a pattern that's wrong before adding another layer on top of it
• Review teammates' code with specific feedback, and pair on hard problems when that's the fastest path

Production and Compliance

• Write the tests, read the logs, and take your turn on call
• Handle PHI carefully in logs, non production environments, and support workflows
• Read the security ADRs and ask when you're not sure where the line is

Education

A four year college degree from an accredited institution, or engineering depth that makes one unnecessary. Show the work.

Experience

• Five or more years building production web applications end to end, with real products shipped to real users
• Two or more years of daily AI assisted development with Claude or an equivalent coding assistant
• Production TypeScript and Node.js, including type design and how Node behaves under load
• Modern React in production, and comfort with React 19 and Vite within your first week
• Real PostgreSQL work: debugging slow queries and running schema migrations without breaking things
• Working knowledge of Lambda, API Gateway, RDS or Aurora, S3, and CloudWatch
• REST and FHIR style API design, including resource modeling, idempotency, versioning, and authentication
• HIPAA experience, or the willingness to come up to speed quickly

Preferred

• EMR or EHR work, especially in post acute, skilled nursing, or assisted living
• FHIR R4 resources, bundles, and search parameters, ideally in a FHIR native system
• Hono, tRPC, Drizzle ORM, or comparable modern TypeScript tooling
• AWS CDK in TypeScript, or production Terraform or Pulumi
• Multi tenant PostgreSQL with row level security
• Vitest, Playwright, Testcontainers, OpenTelemetry, and Sentry
• Clear written and spoken English for a team that works mostly asynchronously
• Visible open source work in the TypeScript ecosystem

Travel

As requested and needed, including occasional on site team meetings where we work through product design and architecture strategy together. Integration testing may call for an occasional trip too.
