Alleva builds the EMR that behavioral health and addiction treatment centers run their organizations on — clinical documentation, scheduling, e-prescribing, utilization review, and billing. It is a multi-tenant SaaS platform under HIPAA and SOC 2, so what we ship must be fast, auditable, and dependable.

We are looking for a senior engineer who is expert on the backend and comfortable owning a feature all the way through the UI. You will design and build C# APIs and the background processing behind them, model and tune the Azure SQL databases they depend on, and ship the Angular front-end clinicians use. This is a hands-on role with no direct reports, real ownership of platform areas, and influence over how the system evolves.

•     Design, build, and operate secure, versioned ASP.NET Core APIs serving our web, mobile, and partner integrations — owning service boundaries, data contracts, and the tradeoffs behind them.

•     Build background and asynchronous workloads — scheduled jobs, queue consumers, imports, document generation, billing runs — designed for idempotency, retries, concurrency safety, and graceful recovery.

•     Own the efficiency of the data layer in Azure SQL: schema and index design, query tuning, migrations, and diagnosing blocking, deadlocks, and plan regressions across a large multi-tenant estate.

•     Ship user-facing features in our Angular application, partnering with product and design to reduce clicks for clinicians rather than adding them.

•     Build on Azure and keep application design aligned with cloud architecture — App Service, Functions, Storage, queues, Key Vault, Front Door, Application Insights.

•     Work with our DevSecOps team on CI/CD, blue/green releases, monitoring, and scaling, and add automated performance and load testing so regressions surface before release.

•     Troubleshoot production incidents, lead root-cause analysis, and implement the preventive fix — not just the hotfix.

•     Treat security, privacy, auditability, HIPAA, and SOC 2 as design inputs rather than a final review step.

•     Raise the bar in code review, mentor mid-level engineers, and use AI-assisted development tools to move faster while staying accountable for what you ship.

•     7+ years building and operating production software.

•     Expert C# and .NET — ASP.NET Core Web API, Entity Framework or Dapper, dependency injection, async/await, and concurrent code.

•     Expert-level background job and asynchronous processing experience (Hangfire, Quartz.NET, Azure Functions, Service Bus, or equivalent), including scheduling, retries, idempotency, and observability.

•     Expert Azure SQL or SQL Server — advanced T-SQL, data modeling, indexing strategy, execution plans, transactions and locking, and performance tuning at scale.

•     Expert-level experience with a major cloud platform: Azure strongly preferred, AWS or Google Cloud considered.

•     Solid, productive frontend skills in Angular, React, or Vue.js — enough to own a feature through the UI without waiting on someone else.

•     Strong system design judgment: distributed systems, API design, asynchronous workflows, fault tolerance, and failure recovery.

•     A track record of diagnosing hard performance problems using profilers and APM tooling such as Application Insights, Datadog, New Relic, or OpenTelemetry.

•     Git and pull-request workflow, CI/CD, and automated testing as normal practice.

•     Clear communication and a record of independently owning systems from design through production support.

•     Healthcare technology — EMR/EHR, HL7 or FHIR, e-prescribing, claims and revenue cycle, or behavioral health workflows.

•     Work in a HIPAA, SOC 2, or otherwise audited environment.

•     Multi-tenant SaaS, including tenant isolation and zero-downtime schema migrations.

•     Angular specifically, including RxJS and NgRx or a comparable state library.

•     Infrastructure as code (Terraform, Bicep, ARM), Docker, Redis or distributed caching, and load testing with k6, JMeter, or NBomber.
